What We Accept
Megacare is a high-acuity provider. Our clinical depth — RN supervision, trach/vent-trained LVNs, point-of-care tablet charting — is built for the cases most agencies will not take. If we cannot staff safely, we tell you on the first call and, when possible, suggest an agency that can.

How to Send a Referral
Phone is the fastest path for urgent discharges. Secure fax is the preferred method for discharge orders, H&P, and POC paperwork. Please do not send patient identifiers by unsecured email.

What to Expect
The protocol is the same every time — built so you can hand off the patient and move to the next discharge without guesswork.
Referrals received during intake hours are typically acknowledged the same business day. If we cannot safely staff the acuity, we tell you on the first call and do not waste your time.
A registered nurse reviews the H&P, discharge summary, active medications, equipment needs, and plan of care before we accept. Acuity-match is non-negotiable.
For Medicaid managed care, we work directly with the MCO utilization management team to confirm authorization before the first shift. No surprise denials.
An intake nurse visits the family in the home to complete the environmental and equipment walk-through, then we coordinate start of care with the family, MCO, and referring physician. Urgent trach/vent discharges are expedited.

We work with major Greater Houston hospital systems and Texas Medicaid managed care plans across the metro. Your case management team may already have us on file.
Placing a Complex Case?
Our intake team is available during business hours and on-call for urgent discharges. When hospitals discharge complex cases, they call us.
