Requesting Your Health Care Records
You have the right to request copies of your health care records from Megacare Home Health Services. Records requests are processed in accordance with Texas Health & Safety Code Chapter 181 and federal HIPAA regulations (45 CFR §164.524).

What to include in your request
- Your full legal name
- Date of birth
- Phone number and/or email address where we can reach you
- Dates of service, or the specific records you are requesting
- Whether you would like electronic or paper copies
- Where we should send the records (your address, secure email, or another health care provider)
We may ask you to sign a Release of Information (ROI) form before releasing records. If you are requesting records on behalf of another person, you will need to provide proof of your authority to receive them (for example, a Medical Power of Attorney, guardianship order, or a signed authorization from the patient).
Timing and fees
We will respond to your request within the timeframes required by Texas and federal law. Copying or delivery fees may apply in accordance with applicable fee schedules, and we will inform you of any fees before processing your request.
